What nursing model is used for planning care?

Braeburn House House provides a holistic and person-centred approach to recovery, in order to help our service users reach their full potential. Recovery Star tools are used to measure and monitor recovery.  This model of care provides the overarching philosophy for the service, the principles of engagement with individual Service Users and the processes and objectives of care.  It provides the framework for our interventions with service users and our collaboration with other services to provide a comprehensive pathway of care through inpatient care and into the community. This will be achieved through: Formulation-based management and intervention and workforce training and development.  Our model of care is underpinned by a set of coherent objectives to achieve key milestones in the understanding and managing of service user difficulties and the impact these have on day to day behaviour, interactions with others, and engagement with care providers.
